Van Reservation

To request the use of our church vans, please complete and submit the form below. Once approved, you will be notified, and your reservation will be placed on the church calendar. 

  • Reservation requests must be submitted at least a week before the date the vans are needed.
  • Drivers must be members of FBC Ellijay, must be at least 24 years of age and have a clean driving record. A copy of the driver's license of the person driving the van MUST be on file in the church office. 
  • The van keys may be signed out in the church office during regular hours (M-Th 8:30-4:30, F 9:00-12:00) unless other arrangements have been made.
  • Please read our VAN POLICIES before completing the form.
